Our membership registration form powered by TraitWise asks users to give us contact information (like their name, email, and postal address) and information about their health.

Surveys
Our online surveys ask visitors for contact information (like email address), and demographic information (like zip code, or age).

We believe that this information will ultimately help doctors better understand rare neuro-immunologic disorders and the people who have been diagnosed with them.
