Our membership registration form powered by TraitWise asks users to give us contact information (like their name, email, and postal address) and information about their health.

Surveys

Our online surveys ask visitors for contact information (like email address), and demographic information (like zip code, or age).

Demographic and profile data is also collected at our site. We greatly appreciate your willingness to provide us with this very personal information. We believe that this information will ultimately help doctors better understand rare neuro-immunologic disorders and the people who have been diagnosed with them. We believe that this information will ultimately help our members.

The information shared with us will not be made public at any time without your permission.  Data we collect maybe analyzed as de-identified data and only aggregate results reported and used to learn more about the disease and make reasonable conclusions about the population that may be helpful in designing registries and clinical trials. The TMA does not share or sell patient data and other confidential information with third parties for any purpose.
